Put fill neck into hand warmer center hole, fill no more than a third of the measure at a time, or get backfill / spill on the side of the hand warmer.

Earlier I got it lit and all seemed ok, it never got very hot despite my leaving it outside of the sleeve, then after about 45mins it died and went cold.
